Most importantly, we strive to develop care plans that reduce risk and enhance the quality of life of every client we serve.
Additionally, our agencies coordinate with Medicare-affiliated skilled home health and hospice providers. Through our agencies with nursing services, we also provide:
- Care Coordination — Physician and provider visit coordination, vitals monitoring (where state regulations allow), medication delivery, home safety assessments and other services as needed.
- Referral Services — Our team also helps to initiate referrals to other professionals in the community for supportive services.
